Instructions

This is a 4-part assignment:

1. Create a graphic of Leavitt’s Diamond for the program in which you are placed for your field internship. Use the template as discussed in class.
2. Write a one-page description of the graphic elements referencing the readings for class.
3. Prepare a one-page logic model and a one-page description of the logic model referencing the readings for class.
4. Write accompanying paper of a total of 5 pages in APA format. The paper will briefly describe the program (one paragraph) and explain the ‘if-then’ connections in the logic model pointing out any discrepancies and making recommendations for programmatic changes.
